High Dose EPA/DHA Fish Oil, Cholestyramine, and the Herx

I've been having a difficult time getting started on cholestyramine therapy to start removing the biotoxins from my system. As a part of my treatment plan, I first have to remove myself from sources of biotoxin exposure, primarily mold. Once that is done it's time for toxin removal from the body. Cholestyramine is a bile acid sequestrate intended for use in lowering cholesterol. Coincidentally, cholestyramine's unique structure and positive molecular charge forms strong bonds with negatively charged biotoxins, be it from mold, lyme, or other sources. As such, this makes Cholestyramine an excellent vehicle for binding and removing biotoxins. Cholestyramine has been used by the U.S. Government to treat military personal exposed to neurotoxic agents because it is so effective an has few side effects. Cholestyramine or (CSM) for short, does not leave the digestive tract and does not make it's way into the blood stream. Simply put, CSM binds to biotoxins which have been processed by the liver and, excreted into the bowel via the bile ducts. Once CSM comes in contact with the biotoxic bile, it binds to it tightly and holds on to it until It can be excreted when you have a bowel movement.

So what's the problem with CSM? Shouldn't removing toxins from the body in this way be pretty easy? It should, but it isn't always that easy. CSM causes gastrointestinal symptoms and constipation, not good things to have when your goal is to pass the bound toxins as quickly as possible. But there is a second reason why CSM can make things worse. Biotoxins from Mold and Lyme sources are fat-soluble meaning that they predominately reside in your adipocytes (fat-cells). If you envision CSM as the guy who opens the flood gate and puts a fishing net across the way to catch any fish that come through, you can understand what CSM is doing in your body. Once CSM starts binding to the biotoxins in your enteric system and ushers them out of the body, there is now "new space" for more biotoxins to move on down the line. Think of this process like a bucket that's too full. Once you dump out some of the contents, you have room for more. In a rudimentary way, that is what happens in our bodies. So if we know that the biotoxins are stored in the adipocytes (fat cells) and that they are migrating from those cells into the blood, and then being processed by organs such as the liver into the bile and into the enteric system, the delivery channel becomes clear. Now you see why it is important to have a bile sequestrate ready and waiting for the newest batch of toxic-bile to be delivered, bound, and ushered away.

So far so good, right? In a perfect world perhaps. In people with Lyme toxins another key process can take place. As many of you probably know, CSM has been shown to cause "Herxheimer" reactions in many. After reading through Dr. Ritchie Shoemaker's book Desperation Medicine, I now understand to a small degree what is happening to those of us who get worse once we commence this kind of treatment. If we go back to the elimination channel of biotoxins talked about in the previous paragraph, we can look at a particular process that occurs specifically in those with Lyme toxins. The biochemistry of this process is a bit complex, but it's not impossible to understand. The presence of biotoxins (in this case those from borrelia burgdorferi) bind to the insulin receptor sites on adipocyte cells (fat cells). Once there, a switch is turned on which triggers and alters DNA transcription of mRNA, causing the adipocyte to produce a powerful cytokine called Tumor Necrosis Factor-alpha or (TNF) for short. The role of TNF in autoimmune and inflammatory disease has recently drawn a lot of attention as it should. In the case of innate immune response and biotoxin illness, it is also significant. So what does this mean? Simply, once the biotoxin "parks the car in the adipocyte's garage", the adipocyte's becomes a TNF-producing factory. Envision millions of cells in the body producing this powerful cytokine. In controlled amounts, TNF and other immunoregulatory cytokines such as the IL (Interleukins) IL-1, IL-8, etc. are helpful and a vital part of the immune systems response to infection because they help to induce fever, synthesis of phase proteins by the liver, and activation of T & B lymphocytes. However, in much higher quantities, these cytokines are responsible for many of the pathological symptoms we see in inflammatory conditions. Simply put, too much TNF will make you feel absolutely awful. Such an influx of TNF is often called the "TNF storm". Cytokine storms can be dangerous and in some cases cause damage to various organs. An example of this is the neurotoxic effects of elevated TNF causing damage to the rentromedical hypothalamic (VHM) neurons resulting in hyperphagia, hyperglycemia, hyperinsulinemia, hypertriglycerdemia, and impaired glucose tolerance (IGT). High concentrations of TNF are also correlated with the induced apoptosis of neuronal cells.

So what can we do to reduce or stop a TNF storm if we have lyme toxins in our bodies and have to start removing them with CSM, Welchol, or other bile sequestrates? Thanks to doctor Shoemaker's research we have an answer. As I said before, the biotoxin, in this case from Lyme, binds to the insulin receptor site on the adipocyte in much the same way as insulin itself would do. If we can block the insulin receptor site, we can block not only insulin binding, but also biotoxin binding as well. That is exactly what happens when one takes the pharmaceutical drug, Pioglitazone (Actos). So Actos can be used to significantly blunt or mitigate the symptoms of a TNF storm resulting from stirring up latent biotoxins in the bodies fat stores. With this information at hand, we can see why pretreating with Pioglitazone prior to CSM commencement is important. So are there problems with taking Pioglitazone? In my case there seems to be. Because the function of Pioglitazone is to help normalize insulin in type 2 diabetics, those with insulin related problems and low-leptin as a result of biotoxin illness can have adverse reactions to the drug. In patients with low-leptin (as myself) and those who are very slim (as myself), Pioglitazone can cause blood sugar levels to drop too low.

What other options are there for people who need to pre-treat before starting CSM or sequestrate therapies? High doses of omega3's Eicosapentaenoic acid (EPA) and Docosahexaenoic acid (DHA) can help. In keratinocytes (predominant cell type in skin) and fibroblasts (think collagen), AA, EPA/DHA reduced basal secretion of IL-8 (Interleukin-8, a pro inflammatory chemokine produced by macrophages) by 66% and 63% respectively. http://www.ncbi.nlm.nih.gov/pubmed/15654981 It becomes obvious that EPA/DHA plays a key role in reducing inmmunoregulatory cytokines including IL-10, TNFalpha, and INFgamma. These oils also have strong regulatory effects on B-Lymphocyte gene transcription. Macrophages (white blood cells in the tissue produced by the division of monocytes), and monocytes themselves are also responsible for producing key cytokines, TNF, IL-1 and IL-6. High doses of omega3's have been shown to reduce capacity of monocytes to synthesize IL-1 mRNA. Basically, EPA/DHA does what Actos does, but also has direct affects on the immune cells which regulate and release these inflammatory cytokines. Pharmacological drugs like cortocosteroids and cyclosporin are traditionally used as inflammatory controllers in this case, albeit they have side effects. That being said, EPA/DHA in high concentrations can achieve similar results, with little risk of side-effects, and have many other benefits for the body especially for the circulatory system. Bottom line, high dose omega3's EPA/DHA and those from Flax Seed have high potential for reducing cytokine progression. EPA/DHA has an amazing ability to suppress platelet activating factor (PAF) which is a potent platelet aggregator and leukocyte activator with links to RA, arthritis, asthma, endotoxic shock, and acute renal transplant rejection. http://www.ncbi.nlm.nih.gov/pubmed/15654981

So enough with the nitty-gritty, where am I at? Like I said, I started CSM therapy and got much much sicker. My inclination is that this was due to some sort of TNF-storm as a result of mobilizing latent biotoxins from my fat cells. I've since stopped CSM and was instructed to take high dose EPA/DHA to pre-treat before starting a new bile sequestrate, Welchol. Welchol is about 25% as effective as CSM, but is gentler on the body. Nordic Naturals is a good quality fish oil that I'm using right now to start. The doses may be different depending on multiple factors such as weight, etc, so I cannot say with certainty that my prescribed daily dose is right for anyone else. That being said, I need to try to get up to 2.4g EPA and 1.8g DHA daily for 8 days prior to commencing Welchol therapy. This works out to roughly 8 capsules per day. I am assuming that at this high dose, the fish oil can really start to suppress inflammatory cytokines, increase low VEGF (vascular endothelial growth factor), and mitigate increased TNF symptoms. As for now, I'm having trouble working my dose of the fish oils up as they seem to make me feel worse. I suspect that the high fat concentration of the EPA/DHA capsules is stimulating bile flow (full of toxins) in my body and that could be making me feel worse after taking them. I've also read that high dose EPA/DHA and Chlorella functions similarly to Actos and CSM. Perhaps there is some factor unbeknownst to me allowing the EPA/DHA to cause further detox? That would be a bit contradictory however. So, I will wait and see. Slowly work my doses up until I can tolerate the 8 capsules per day. Wish me luck!

  1. While most people with severe biotoxin illness (especially when manifesting as ME/CFS) cannot take any cholestyramine at all, those of us who are pursuing what has been called "Extreme Mold Avoidance" seem to be able to take it without a lot of problem.

  2. It is essential to get mold out of your environment first, major mold exposure at least. I found using cholestyramine and welchol not especially useful and overly expensive. High heat dry saunas or steam saunas are the BEST choice for detoxing biotoxins of all kinds including mold exposure imho. It simultaneously kills pathogens and detoxes the biotoxins as well as burns fat and cleans the blood. So over time your toxicity levels get lower and lower and your pathogen load also gets lower and lower. But you bypass much of the toxicity overload experience if not all of it. It can take daily sessions in a sauna though if toxicity is high, working up to at least an hour with plenty of electrolyte water.

A correction about the Herxheimer reaction, which is straightforward to explain: if an infection is caused by something that secretes a toxin, an antibiotic that kills the infecting organism can cause a "dumping" of all of the toxin that was in that organism. This kind of surge of toxin produces the symptoms that are called the Herxheimer reaction. No need to invoke any complicated biochemistry.

    Also, because cholestyramine causes the elimination of toxin that was already released, it is not at all clear how cholestyramine could cause a Herxheimer reaction. It could be that cholestyramine is enabling an otherwise ineffective antibiotic to be effective, in which case it is the antibiotic that is directly causing the Herxheimer reaction.

  4. If removal is to be performed when users are still present in the building, it is usually necessary to relocate some users temporarily. Typically, the part of the building from which asbestos abatement is being removed has to be sealed off in order to prevent contamination of the other areas. Methods of sealing off an area often include the use of Polyethylene film, duct tape and negative air pressure machines which are fitted with filters. The idea is that the contained area is pulling fresh air in as to not allow asbestos fibers into the surrounding environment.Only a special vacuum cleaner that's designed for asbestos containment (class H) can be safely used when cleaning up during and after asbestos removal. Ordinary vacuum cleaners cannot be used, even those fitted with a HEPA filter.[1] An ordinary vacuum cleaner will expel the asbestos fibers into the room air.

  6. This post is several years old. I would love to know what ended up happening for you! I have lyme (pursuing natural therapies) and mold toxicity, and started CSM (4g/day) 2 weeks ago and it just makes me feel so awful (exhausted, muscle aches and tension, severe headaches), I am not sure I can continue on it. Did the fish oil pre-treatment and the Welchol go easier for you and did you get results?

    1. I also recently started CSM for mould toxicity and have found it amplifies my symptoms. I manage on it for a week or two and then have I come off it because the headaches and fatigue become too much. I have noticed a huge improvement since before I started, however I just can't seem to stay on it for long enough. Currently taking one dose a day and going to try and gradually increase this to four.

  8. Your issues with omega 3s might also lie in the fact that you need omega 6s instead. I did horrendously on omega 3D and they made me far worse. Took some Evening Primrose Oil and felt like a new person. Omega 6s might be considered bad but when your body's chemical system is out of whack it might need what you wouldn't normally think of as good.

  11. Hi everyone, I'm just wondering what kind of cholestyramine is talked about here? I went to my GP who's more than happy to help me (after nearly 2 years of trying to figure out what's been wrong with me even though I've told her since day one it's mould, shes one of those docs who were never taught and therefore unaware & doesnt believe ...although now is starting to believe) and give me prescription but told me there are two forms, pills/tablets, and sachets. I've got a script for the sachet form, but dont want to buy it before I know that's the right one. If anyone who knows could clarify I'd really appreciate that.
    I concur (based on my own experience), starting any kind of treatment without eliminating the exposure (leaving the contaminated area), would be very likely futile. Also, your belongings must be either very thoroughly disinfected, or thrown out. Mattresses, sofas, upholstered chairs etc ...simply anything and everything that cant be washed must be thrown out due to mould spore and VOC's contamination through the air ...thats out of question. Clothes, blankets, quilts, pillows, and everything else that can be wash, should be washed in ammonia (the only thing that allegedly - I can confirm it certainly does something, as I've recovered about 85% so far, and kept everything from my old apartment that could be washed - kills mould spores and eradicates their byproducts - biotoxins, by neutralizing them {something to do with the pH according to one lab.chemist I consulted}), I purchased industrial strength 25% ammonia and added it about 1/3L into every load of laundry (wearing gloves and full face respirator, plus it can irritated exposed skin - at least wear long sleeves and pants, if you dont have a rubber onesie) until every single item I owned was washed.
    I can confirm, Omega3 (fishoil) must have something to do with helping rid of the biotoxins, because I have been on it for the past at least 6-7 years (daily) for general wellbing, and as soon as I left the mould infested apartment (mould discovered under the carpet under the bed, and on the back of some wooden furniture due to the increased humidity from building defects that caused leaks into our bedroom) my symptoms that I had for years starting disappearing within weeks without any special medication. And I'd say my case of poisoning would have been (and probably still is to some extent) one of the worst ones. I lived in that apartment for 9 years during which I was diagnosed with chronic cough resembling asthma, skin condition resembling psoriasis, hairloss, rhinitis, chronic sinusitis, damaged hearing (hearing cracking sound in my ears triggered by certain frequencies - diagnosed as a certain muscle partial dysfunction that is supposed to protect the ear drum from loud noises), nosebleeds, anxiety, aggressiveness, red irritated eyes, damaged vascular system, alcohol sensitivity (red spots on face, neck, chest, shoulders, upper back when I drank strong alcohol, like vodka for example, too quickly), lower back & shoulder joint pain... etc list goes on. Obviously the onset was slow over the years, and not all symptoms at once. continued in the next post due to the character count limitation...

    I've managed to boost and speed up my recovery by taking glutathione, and astaxanthin ...reportedly the most powerful antioxidants on Earth, as well as general antioxindants such as Vit A, E, high streght 1000mg Vit C with bioflavanoids (5 pills a day) high streght garlic pills, also some melatonin, and selenium, and to fix the digestion and leaky gut syndrome (causing broken dilated dark capillaries) pure aloe vera leaf extract from the chemist, Vit K2, bone broth (for collagen/gelatin) and probiotics (Inner health plus, diluted Apple cider vinegar, sauerkraut). Within weeks my alcohol sensitivity was completely gone (after about 7 years of asking myself where the hell it came from and why), my constantly red irritated eyes and occasional blurry visions is back to normal. My so called asthma and psoriasis is 95% percent gone, I started having dreams again after 8 years of dreamless nights, sleeping 100% better without waking up in the middle of the night and feeling constantly run down and tired.
    My joint pain has receded about 70% and still getting better. My hearing has improved about 50%. My chronic sinusitis, anxiety, aggressiveness, nosebleeds ...all completely gone.
    It might be worth mentioning that I have been going to the gym for the past about 14 years, so I've been pretty fit and my body probably in a good shape to repair itself, once it had nothing to be constantly fighting off anymore. Also I have been going to the sauna each time I am at the gym (3-4 times a week) that must be helping with the toxin release 7 removal as well.
    Also, before I moved out of the apartment, I purchased air filtration device, IQ Air HealthPro 150, which has a hyperhepa filter (which catches mould spores) and activated carbon filter (which eradicates airborne mycotoxins). After having used it for about two weeks my skin condition (would-be psoriasis) that I had for about 2.5 years prior constantly no matter what steroid creams I was using, suddenly disappeared without any medication. That gave me the final confirmation that there was something wrong with the apartment. Ever since then I have been on the road to recovery knowing what most people don't. I hope sharing my experience will help someone out there.
    So again, if anyone could let me know if I've got the right form of cholestyramine, that would be great, thanks in advance.

      Am I to understand correctly that you did all of that repair WITHOUT the cholestyramine? This healing you had was all from nutrition and supplements with lifestyle changes? I am really having a hard time deciding which path to take for our family of 8. My six kids have been very ill and I have a two year old I am very concerned about. The problem in the U.S. is that our docs don't want to consider natural remedies and know dick about nutrition and supplements to begin with. They also do not want to deal with a toddler because the treatments they use here are too aggressive. I am looking for a naturapath doc that can help...time will tell. ~Rachel

    2. Sorry to hear about your kids. Yes, I have been on a very strict regimen that these days includes even a slightly unusual practice - but it definitely helps me. Read below. I imagine it would be very hard to follow with young kids. However I have been doing it all myself. If a trained practitioner were to be involved, perhaps it would seem a bit more doable even with the kids. But not the two year old.
      You're correct, I have done all this progress in my recovery WITHOUT any cholestyramine (CSM) at all. Having said that I have even been checked & diagnosed to have the gene that reportedly makes me vulnerable to mould and its toxins, as my body isnt naturally capable of removing them on its own, therefore people like me (reportedly estimated at about 25% of population) get sick more easily than the lucky rest of us. Thats a theory described by Dr. Shoemaker, that he incorporated in his treatment protocol. Meaning if I wasnt doing the right thing, my recovery should be very slow, if any at all, considering my body should not be able to heal itself. Thats just my opinion/conclusion though.
      Recently I have gone off the idea of using CSM at all, as I have found a mould trained practitioner here in Sydney - recently certified under Dr. Shoemaker in the U.S., who follows his protocol. That includes taking CSM and BEG nasal spray. She told me there are many possible, even though very unlikely, side effects that CSM can have in some individuals. I have done some research and found that there has been even one death linked to taking it, vision problems, and more. In animal trials causing tumour. When I asked her about it, she did not deny, however said that the form of CSM she prescribes is from a compounding chemist where they prepare it without aspartame - the ingredient that is commonly found in the generic pharmacy grade of CSM, which is blamed for the tumours. After hearing that I said to myself I am not that desperately ill anymore to take the risk, however big or small.
      What I have been doing recently, over the last 5-6 weeks, I ve gone of the Glutathione supplement, as I have found out that being on it for too long can cause your body to stop the production of its own.
      Instead I started taking glutathione precursors to encourage my body to create its own - NAC (N-Acetyl-L-Cysteine) ... that is the name of the supplement that contains the most important one that is scarce in your every day diet. Then Glycine, and Glutamic acid that I get from my gym protein supplement, otherwise any protein rich red meat is a good source. Look up other sources.

    5. I have been told by a pharmacist that it's good to take Selenium with them as well, as it has something to do with the production of Glutathione. Havent done any research on it to determine if it's true or just the pharmacist's marketing ploy to make me spend more, as I dont care. I have been taking selenium for a few months now anyway, as it's a powerful antioxidant, along with a heap of other supps mentioned in my previous post. Some of the natural ones that I may or may not have mentioned before are turmeric (natural and supplement form) horseradish both white and red(natural), garlic (natural and supplement) and to promote the production of glutathione also quercetine (supplement)
      Also, I started doing coffee enemas, which is the previously mentioned unusual practice (which reportedly raise the body's glutathione level by 600%-700%, yes, thats right 600-700%, apparently its also used as an alternative/natural cancer treatment - since I started doing them - initially twice a week, now just once ,double session back to back) combined with taking charcoal tablets to bind the released toxins and stop them from reabsorbing ...this I hope will at least partially do what CSM does without the possible side effect. I am still noticing improvements on a weekly scale. Especially with the residual joint pain in my shoulders and back, which I would say has jumped from the previous 70% to about 90-95% improvement.(there are moments in my days when I still feel a bit of discomfort, but most of the time there's no pain at all) It has fixed my metabolism, I eat more and lost weight (not much, but for someone who works out and is generally fit, still an improvement), skin has cleared even more, I'd say 100%, if before it was 95%, my damaged vocal cords have started repairing themselves - I used to have very annoying voice changes through the course of my day, which the doctor attributed to possible polyps on my vocal cords that are common in mould patients. Lately my voice has been returning back to normal and the changes are less frequent ...all this since I started doing the enemas. I have also done a couple of garlic enemas.
      As a substitute for the BEG spray (which also is a prescription item and may have some undesirable side effects aside from reportedly causing pain when used to eradicate the so called MarcOns from your sinuses, which negatively affect/deactivate some gland in our forehead (front part of the brain) that is supposed to produce a certain hormone (or something else that I dont remember) that helps your body to fight and remove the toxins) I started doing sinus flushes before I go to bed every night - product called FESS Sinu-Cleanse here in Australia, its a non medicated sinus flush containing Sodium hypochloride, sodium bicarbonate and Xylitol. That's been helping me to get rif of my residual chronic sinusitis - having a bit of stuffy nose every morning when I wake up. Since I started doing these a couple of weeks ago, my nose is about 93-98% clear every morning. And it has been getting better every week.
      Bottom line ...one good doctor once told me that our body has an amazing ability to repair itself, we must just find what it needs and give it to it. I have been living by that ever since and I would never look at the commercial drugs, the so called medicine, ever again for help, nor have I had a need to. I dont even go to GPs anymore, when you do the right thing by your body, you wont need one, as you wont get sick. And If I ever need help with something more serious that requires a deeper knowledge & understanding, I always find the best solutions combing my online research & other peoples' experience with consulting a good reputable naturopath. An every day GP would not know the answer anyway, and in the past I had always ended up being referred to an integrative GP/naturopath anyway.

    6. Good luck, and if it doent help you, hope my response at least gives you some hope. But you definitely need to end the exposure, if you havent done so already. Thats the first step that if skipped it renders every other attempt/action completely futile. If you have already moved out of the place that made your family sick, and you are not seeing/feeling any kind of improvement/relief, or your symptoms improved a bit but keep returning, even if on a smaller scale, and you have done the obvious things like properly disinfecting your clothes, pillows, quilts etc etc simply everything , replacing your beds mattresses and all bedding where you lie and breath every night for at least 8 hours if there is a chance the contamination reached the bedroom, and everything with a fabric surface that cant be washed in ammonia - i know its gonna be expensive, especially for a family of 8, then there is a chance you again are being exposed in the new place without even knowing it. It happened to me twice, after leaving the or
      iginal place. It made me furious realizing it at the end of each tenancy that I was still around mould for another two years after leaving the original apartment. My second apartment turned out to have mould under the carpet in the bedroom, and a leak that could not be fixed even after 4 attempts. I lived there for 1.5 years, during which my symptoms somewhat improved but kept coming and going at a less frequent rate. Compared to the first place. The third apartment where I spent about 7 months also reportedly had mould history, that they thought they had fixed. Here my symptoms again improved a bit, but others (new ones) started appearing that I never had before - that taught me different type of mould - different symptoms. However the mould was not obvious/visible to the naked eye. Nothing serious, just a bit of mould on the wall that I occasionally wiped of that could not make anyone sick, but weird stuffy smell most of the time in the en suite bathroom and bedroom. Suspicious crack between the dry wall ba
      throom tiles that could only be caused by movement of the wall. And one of the possible reasons for wall movement is moisture as it soaks up. Could not locate any mould, so suspected there must have been something inside the dry wall. Did an air sample test that reported raised levels. Started sleeping temporarily in the spare room - the other end of the apartment - and thats when the real recovery miraculously started with a couple of weeks. Moved out within another few weeks into the fourth apartment which I thoroughly picked and inspected. I have lived here for a few months now, and I have been seeing nothing but improvements on all fronts within the first couple of weeks and ever since.
      This experience has made me realize how common mould is and how many people out there are getting sick on a daily basis and developing cancers as a result without the tiniest clue why providing they never find the mould and realise what it is thats making them sick remainig to live in the same environment. It makes me wonder how much less cancer ther would be in the world if there were no defective water damaged buildings and people lived in a cleaner environment.
      And the landlords let the sick tenant out and the next one in without any care whatsoever.

If you have not watched this and want to understand the topic better, watch this doctor's lecture, its long, but well worth it ... at times it may feel like he's talking about irrelevant facts, but it will all make sense in the end... theres also his contact email there as he still offers consultations even though he's retired. He's based in the US.
      The first video link shows his email contact at the beginning, the second lecture seems to be the video of the same lecture as it starts the same, but its 15 min longer, not sure why, as I only watched the longer one. He explains everything there about becoming sick, symptoms, to treatment and recovery, plus more. I came across this video at the beginning of discovering the mould problem so it taught me a lot and thats where my journey began. Just so you understand, this doctor may still practice the aggressive methods you and i dont like, i dont know, I never contacted him, but his lecture explains everything that happens, how it is, and what needs and shouldnt be done. But he said there that he takes a very personal and individual approach to every one of his patients, and that could be understood as the opposite of the CSM based one size fits all treatment.

  13. Thank you for the great article. One option that is slowly working for us is very low-dose CSM. We moved out without our belongings from a house that had all the worst molds on ERMI. Remediated and moved back, rescued/remediated some belongings, so getting low amounts of exposure. I start at 1/8 teaspoon twice a day working up to higher doses three to four times per day. With fish oil, 3g/day, we got up to 3/8 tsp 3-4 times a day, with manageable side effects, and significant improvement for my son and I (he's probably got bartonella too and has had herx like reactions to even mildly antibacterial herbs). Every time we stop, we have to build our way up back again. My goal is to work up to higher doses but not at the cost of worsening health. One step at a time.
